Patient Safety& Risk Management (PSRM) Nurse will be responsible for the professional management of a comprehensive health and safety/risk management program and for the introduction and/or implementation of measures to ensure the EFRS' operate under prevailing state and federal patient safety guidelines.

QUALIFICATIONS:

  • 1-5 years of Patient Safety and Risk Management experience.
  • 1-5 years of management experience, preferably in a clinic or healthcare setting.
  • Demonstrate a high degree of leadership, motivation, self-direction, and initiative to achieve goals of the mission; Self-motivated, able to work independently and stay organized; and demonstrate the ability to collaborate with stakeholders.
  • Appropriate licenses and certificates for a Licensed Vocational Nurse (LVN).
  • 3-5 years Healthcare Quality.
  • Patient Safety or Healthcare Risk Management certification
  • Ability to leverage statistical tools and techniques to analyze, interpret, display, and present data.

ESSENTIAL FUNCTIONS AND BASIC DUTIES

  • Report to the sector Program Manager for administrative duties and responsibilities, as assigned.
  • Undertake Patient Safety Risk Management duties as assigned by the National Patient and Quality Management Director, Deputy Director, and local Sector Program Manager.
  • Keep up to date with health & safety legislation and guidelines and advise management on legislative developments which may affect Safety, Health and Welfare at Work.
  • Establish appropriate structures and processes in relation to risk reporting, incident reporting and Health and Safety within each EFRS.
  • Review and ensure compliance with LSGS, contract partners and government policies/procedures/guidelines, in consultation with relevant personnel and with due regard to prevailing legislation and make recommendations to the National Patient Safety-Quality Management Director, as necessary.
  • Be onsite 75% of the time to ensure compliance with all training requirements, adherence to policies/procedures and work-related safety requirements.
  • Coordinate the development and review of relevant/required risk and incident report policies, procedures and guidelines.
  • Monitor trends and assess patient safety within the EFRS. In conjunction with appropriate staff, assist in identification of potential risk through risk assessment, analysis of accidents/incidents and near miss incidents, periodic safety inspections and/or audits. Make recommendations to reduce, prevent or eliminate risks identified.
  • Conduct monthly training for clinical staff within the EFRS.
  • Oversee any vaccination clinics/protocols, implement/track training,
  • Maintain a risk audit database and liaise with management in relation to issues arising.
  • Provide quarterly and ad hoc reports to management, as necessary
  • Highlight trends in relation to accidents/incidents and elevate to NPSRM Director/PMs
  • Liaise with the federal risk management and patient safety subject matter experts (SMEs) on aspects of Health, Safety and Security.
  • Perform and submit Medical Quality Management reporting requirements to include training statistics, Monitoring & Evaluation results, incident reports and/or any other medical quality concerns/issues to NPSRM.
  • Conduct risk management training and perform respiratory protection requirements such as fit testing.
  • Conduct point-of-care testing training and ensure compliance with quality control tasks.
  • Promote an awareness of health and safety in the workplace.
  • Collaborate and participate in medical quality management and patient safety meetings/ initiatives, if applicable, to share and benchmark best practices.
  • Other risk management related duties as may be assigned from by the Sector Program Manager and in collaboration with the National Patient Safety - Quality Management Director.
  • Promote and maintain a safe working environment for all staff to ensure that all Risk Management, Health and Safety strategies, policies and procedures are implemented.
  • Oversee the vaccine program and participate in vaccinating residents with the medical staff
  • All other duties as assigned.

PHYSICAL DEMANDS

  • All candidates must be physically able to perform all strenuous duties entailed in this position. Candidates will be able to exert themselves under rigorous environmental conditions., These physical demands are including but not limited to pulling up or repositioning patients, lifting supplies, bending over to perform certain duties.
  • This individual must also be able to maneuver throughout small hallways and many stairways either in response to emergencies or to conduct daily responsibilities.
  • This candidate will be able to sit and/or stand for long periods of time. Be physically active for long periods without getting tired or out of breath. Coordinate movement of several parts of the body, such as arms and legs, while the body is moving such as: stooping, bending, lifting, pulling, and twisting. Able to walk long distances up to and including 200ft over uneven dirt grounds.
  • Candidates will possess the ability to work irregular hours; and will be flexible to change location within a sector when asked.

REQUIRED

LSGS requires employees to be fit tested at the following times and with the same make, model, style, and size of respirator that they will be using. All candidates will ensure that they will partake in a fit-test that will be administered using an OSHA-accepted qualitative fit test (QLFT) protocol. The OSHA-accepted QLFT protocols are contained in Appendix A of the Respiratory Standard (1910.134).

  • Whenever a different respirator facepiece (size/style/model/make) is used.
  • Whenever visual observations of changes in the employee's physical condition that could affect respirator fit. Such conditions include, but are not limited to, facial scarring, dental changes, cosmetic surgery, or an obvious change in body weight; and
  • Upon employee notification that the fit of the respirator is unacceptable.
  • Before being allowed to wear any respirator with a tight-fitting facepiece and at least annually thereafter
